How to review Incident reports

Open the incidents list, read a report, and start a new incident when you need to file one.

New Horizon Experience. This article describes the New Horizon Experience, which is available now. As of September 28, 2026, New Horizon is the default starting point on a computer or laptop. In the Horizon WebRef mobile apps for Android and iOS, New Horizon is already what you see. If you still have the older layout, choose Switch to New Horizon from the banner or your account menu. You can switch back to Legacy Horizon from that menu any time during the transition.

This article covers the Incidents list for officials, assignors, and coaches who can view incident reports. Use Schedule menu Incidents or Administrative Forms, depending on your organization.


Open the incidents list

Open Incidents. The list shows filed reports for the season window your organization uses. Search and scroll to find a report. On a phone, rows open as cards; on a computer, you see the wider table layout.

Open or file a report

Select a row to open the incident details panel. Choose New Incident when you need to file a new report. That opens the incident form. Print and CSV export are available when your role can use those Utilities actions.


If you still see the older layout

On a computer or laptop, the older legacy layout appears only if you switched back to Legacy Horizon. Choose Switch to New Horizon from the banner or your account menu to return. Menu names below may sit under Administrative Forms or the classic side menu instead of the New Horizon navigation. The jobs you can do on the page stay the same even when the chrome looks different.
