How to submit an incident report

File a misconduct, ejection, or similar event after a game from your personal schedule.

This article covers the File a New Incident Report form in HorizonWebRef. Officials open it from My Schedule after a game they accepted has started, or from Incidents with New Incident. Organization administrators also use this article to turn the Incident Reporting System on, choose who can view reports, and set notification emails.

Use this article when you need to document a rule incident that happened during a game. Typical uses include ejections, misconducts, disqualifications, major penalties, and similar events. It is not the everyday Game Report for scores and ratings. That is a separate Complete game report action. See How to submit a game report.


Turn it on and set it up

Only organization administrators can change these settings. Open System Settings from the menu. On Overview, tap or click the Features & Settings tile. On a larger screen you can also use the left sidebar. Search by number or label (for example incident or 49). Open the Additional Features group if that section is collapsed.

Turn on Incident Reporting System (Feature #49). That is the setting that lets officials file incident reports. When it is off, officials do not see File incident report on their schedule, and Incidents does not appear in the menu. The page saves on its own a moment after you change a setting. Wait for the short Saved or All changes saved message before you leave the page.

Who can view incidents

When Feature #49 is on, Who can view incidents appears under the switch. Submitter and Assignors are always included. You can also include Submitter's partner(s), All Officials, Coaches, Athletic Directors, and League Personnel. People who are checked can open Incidents in the menu and review reports they are allowed to see. People who are not checked still receive email only if you add their address in the notification settings below.

Who gets an email when a report is filed

Assignors are notified when a new incident report is submitted. To copy additional addresses on every incident in the organization, open Preferences on System Settings and fill in Incident notification email. You can paste more than one address. Separate them with commas, semicolons, spaces, or new lines.

To notify different people for one league only, open Game Structure, edit that league, and fill in Incident email recipients. Type an address and press Enter to add it. Those addresses receive notices for incidents in that league.

Your organization also needs teams turned on for incident reports to appear. Feature #49 is meant for organizations that use teams on games.


Open the report from your schedule

Sign in as the official who worked the game. Open My Schedule from Schedule in the menu. The default date range is upcoming games, so a game that already happened may not be on the first screen. Change the date range to include that day (for example Last 30 Days or This Month).

On each game card or row, open Actions (the three-dot button). Choose File incident report. The form opens in a window over your schedule. The window title is File incident report. You can close it and return to the list without losing your place if you have not started typing, or discard unsaved changes if you have.

File incident report appears only when all of these are true: Feature #49 is on, your organization uses teams, you accepted the assignment, the game has started, and the game is not a meeting and is not cancelled. Future games do not offer this action. Horizon does not require an incident report after every game. File one only when an incident occurred.

You can also start from Incidents in the Schedule menu. Officials can tap or click New Incident (the plus button), then select the game.


Enter when it happened and the score

The form heading is File a New Incident Report. A Game Summary line shows the teams and the date so you can confirm you opened the right game.

Choose the period, quarter, half, or inning when the incident occurred. The label matches how that sport divides the game (for example Period). If the game uses a clock, set Time on Clock with the minutes and seconds still remaining. A hint under the clocks says Example: 14:41.

Enter Visiting Score and Home Score as they stood at the time of the incident, not necessarily the final score. The hint under those fields is What was the score at the time of the incident?

Add each offender

Under Offender Details, add every player or coach involved in this incident. The help line says Add each player or coach involved in this specific incident. Until you add someone, the form shows No offenders added yet and Click Add Offender to begin this report. You must add at least one offender before you can submit.

Tap or click Add Offender. You can use the button in the Offender Details heading or the one in the empty box.

Each card is labeled Offender with a number. Fill in:

  • Offending Team

  • Offender Name

  • Number (or choose Coach if the offender is a coach)

  • Brief Infraction Description

  • Rule Number (at least 3 characters)

  • Offending Party Notified? with Yes or No

If you choose Yes, Who Was Notified? becomes required so you can name the person you told. If you choose No, that field stays unavailable. Use Remove on a card if you added someone by mistake. Add another offender if more than one person was involved.

Write the synopsis and submit

Under Synopsis / Summary of Events, describe what happened in your own words. The hint says Provide a clear, detailed synopsis of what occurred. Write enough that an assignor who was not at the game can understand the incident. A synopsis that is too short will not submit.

When the timing, scores, at least one complete offender, and the synopsis are ready, tap or click Submit Report. The button appears at the top of the form and again at the bottom. Horizon asks Ready to submit? and Please confirm you are ready to submit this report. Choose Submit to finish or Cancel to keep editing.

After a successful submit you will see Your incident report has been submitted. You can choose Back to My Schedule or Submit Another if you have a second incident on another game. A submitted report cannot be edited or deleted. If you need to correct something, file a new incident report with the corrected information. The original report stays on file.


Review submitted reports

People who are allowed to view incidents open Incidents from the Schedule menu. The page title is Incident Reports. Search by team, player, infraction, or submitter, then set a Date Range and use Reset if you need to clear the filters. If nothing matches, the list says No incident reports found.

Open a row to read the full report. Officials who only file reports may see the reports they submitted. Assignors and others you checked under Who can view incidents see the reports their role allows. The list uses the organization's current season when you choose Full Season. Change the date range if you are looking for a different window.


If you cannot file or cannot submit

If you do not see File incident report, the game may still be in the future, you may not have accepted the assignment, Feature #49 may be off, or the game may be a meeting or cancelled. An organization administrator can turn Feature #49 on using the steps above. Widen the date range if the game already happened.

If submit does not go through, read the message on the screen. Common cases:

  • You have not added an offender yet. Tap Add Offender and complete the card.

  • The rule number is too short. Enter at least 3 characters.

  • The synopsis is too short. Add more detail about what occurred.

  • The period, time on the clock, team, name, number, or notification answer is missing or invalid.

  • You chose Yes for notification but did not say who was notified.

  • The game is a meeting. Incident reports cannot be filed for meeting entries.

If you close the form with unsaved typing, Horizon asks Discard unsaved changes? Choose Keep Editing to stay, or Discard to close without saving.


How this differs from a Game Report

Complete game report on the same Actions menu is a different form for scores, ratings, and comments after a typical game. Organizations can require a Game Report. An incident report is only for a specific misconduct or similar event. See How to submit a game report when you need that path.
