How to submit a game report

File scores, ratings, and written comments after a game from your personal schedule.

This article covers the Game Report form in HorizonWebRef. Officials open it from My Schedule (the personal Schedule view) after a game they accepted has started. Organization administrators also use this article to turn game reports on and choose which positions must file them.

Use this article when you need to submit scores, star ratings, and comments for a game you worked. It is not the form for ejections or misconduct. That is a separate File incident report action. Assignors who are reviewing reports that have already been submitted use Game Reports.


Turn it on and set it up

Only organization administrators can change these settings. Open System Settings from the menu. On Overview, tap or click the Features & Settings tile. On a larger screen you can also use the left sidebar. Search by number or label (for example game reports or 31). Open the Evaluations & Ratings group if that section is collapsed.

Turn on Require Game Reports From Officials (Feature #31). That is the setting that turns the Game Report form on for your organization. When it is off, officials do not see Complete game report on their schedule, even after a game is over. The page saves on its own a moment after you change a setting. Wait for the short Saved or All changes saved message before you leave the page.

Which positions must file a report

Feature #31 turns the system on. Which officials must file a report is set per position. Open Game Structure from the menu. Search for the position, open it, then open Duties. Turn on Require post-game report for every position that should complete a Game Report after an accepted assignment. Save the position. Officials assigned to a position that does not require a report will not see Complete game report for that game.

When Feature #31 is on and a position requires a report, officials with an accepted assignment see Complete game report after the game has started. Their personal schedule can also show a Needs attention line for missing reports. Until those reports are done, Horizon can hold back some assignment details so the official finishes the report first.


Open the report from your schedule

Sign in as the official who worked the game. Open My Schedule from Schedule in the menu. The default date range is upcoming games, so a game that already happened may not be on the first screen. Change the date range to include that day (for example Last 30 Days or This Month).

If reports are waiting, a Needs attention bar appears near the top. It can say how many games need game reports. Tap or click Show these to narrow the list to those games.

On each game card or row, open Actions (the three-dot button). Choose Complete game report. The Game Report form opens in a window over your schedule. You can close it and return to the list without losing your place.

Complete game report appears only when all of these are true: Feature #31 is on, your position requires a report, you accepted the assignment, the game has started, the game is not a meeting, and you have not already submitted a report for that game. Future games do not offer this action.


Fill scores and details

The form title is Game Report. The top of the window also says Complete game report. A summary card shows the date, time, teams, and location so you can confirm you opened the right game.

Under Details, enter the Final Score. Home Score and Visitor Score use the home and visiting team names as labels. Choose a Game Difficulty of Average, Above Average, or Difficult. Turn on Overtime if the contest went to overtime. Turn on Verbal report to league office only if you already called or spoke with the league office. When that switch is on, a When date and time appear so you can record when you made that call. Choose Yes, No, or No opinion for Would you work this game again?

As you move through the form, Horizon can save a draft on its own. You may see a short Saving... or Saved note. A draft is not the same as submitting. The assignor does not receive the finished report until you tap or click Submit Report.


Add ratings

The Ratings section uses a five-star scale. Rate every category before you submit. Home and visiting Coaches each have Conduct with Team, Conduct with Opponent, and Conduct with Officiating Crew. Home and visiting Players each have Sportsmanship, Quality of Play, and Conduct with Officiating Crew. Spectators has Overall Behavior. Facility uses the location name in the heading and includes Overall Quality, Security, and Officiating Dressing Room or Private Area. Minor Officials & Play Quality includes Minor Officials - Performance and Overall Quality of Game Play.

If any rating is below 5 stars, written comments are required. Comments are also required when you choose No for Would you work this game again? The form will not submit until those comments are filled in.


Write comments and submit

Under Comments, type what happened in your own words. The form says Written comments only. Photos cannot be saved in this report. Do not paste pictures from your camera roll. If a photo is pasted, Horizon removes it. Use bold, italic, underline, lists, or a link if you need them. There is no picture button on purpose.

When the scores, every rating, and any required comments are ready, tap or click Submit Report. The button appears at the top of the form and again at the bottom. After a successful submit you will see Your report has been submitted. Use Return to schedule or Close to go back to your list. A submitted report cannot be edited.

If the report will not open or will not save

If you do not see Complete game report, the game may still be in the future, you may not have accepted the assignment, your position may not require a report, or Feature #31 may be off. An organization administrator can turn Feature #31 on and mark the position using the steps above. Widen the date range if the game already happened.

If submit fails, read the message on the screen. It tells you what to do next. Common cases:

  • Photos or a very long comment cannot be stored. Remove pictures, shorten the text, and submit again.

  • Your sign-in expired. Sign in again, open the report from your schedule, and submit again.

  • The game has not started yet. Wait until the scheduled start, then open the report again.

  • You already submitted a report for that game. The form stays locked.

  • This device could not reach Horizon. Check your connection, then tap Submit Report again.

  • The comments editor did not load. Allow scripts, close the report, and open it again.

If the message includes a short support code, keep that code when you write to Horizon support. It identifies the failure without guessing.


After you submit

Assignors and members who can review reports open Game Reports to browse submitted reports, comments, and summaries. Officials who only file reports may see only their own submissions there, depending on their role.

File incident report on the same Actions menu is a different form. Use it for misconduct, ejections, or similar events. Horizon does not require an incident report after every game. See How to submit an incident report when you need that path.
