How to use Custom Forms

Create organization forms, publish them to members, review responses, and track who still needs to complete mandatory work.

New Horizon Experience. This article describes the New Horizon Experience, which is available now. As of September 28, 2026, New Horizon is the default starting point on a computer or laptop. In the Horizon WebRef mobile apps for Android and iOS, New Horizon is already what you see. If you still have the older layout, choose Switch to New Horizon from the banner or your account menu. You can switch back to Legacy Horizon from that menu any time during the transition.

Custom Forms (the page title is Forms) is Horizon's organization form hub. After you sign in, open it from the main menu under Shared, then choose Forms. Organization administrators build and publish forms, review submissions, and watch mandatory completion. Assignors and observers can review responses when your organization allows it. Officials and other members complete forms that are due, optional, or attached to a game from this hub, from the schedule, or from a login reminder.

Custom Forms is in limited release right now. Your organization sees Forms in Shared only when Horizon has turned the feature on for you. There is no System Settings switch to turn Custom Forms on yourself. When your organization is included, use the steps below. Forms Plus unlocks conditional logic, answer-match notifications, a higher published-form limit, and Horizon AI Summary on the responses board.

Custom Forms is not the same as Custom Profile Fields. Profile fields store ongoing member profile data. Custom Forms collect answers for workflows, compliance, game-day reports, and similar one-time or repeating tasks. Administrative Forms (the account-menu launcher) is a different menu of organization tools and is covered in its own Help Center article.

Open Forms

Forms lives in the main menu under Shared. It is not on the bottom phone bar (Home, Schedule, Availability, Directory) and it is not a top-level left-menu icon by itself — expand Shared first, then choose Forms.

On a phone, tap the menu icon (three lines) in the top-left corner so the main menu slides in. Open Shared, then tap Forms.

On a computer, use the dark main menu on the left. Click Shared to expand it, then click Forms.

When Forms opens, the page title is FORMS. On a computer the subtitle under the title reads Create forms, collect responses, and track completion for your organization. Members who only complete forms see Complete forms assigned to you and review what you already submitted.

On a computer, the same hub uses the wider list layout with the left navigation open.

Organization administrators see the All forms tab and, when they can view everyone's responses, a Compliance tab. Members who are not managing forms see Due, Available, and Completed instead of All forms.

Forms is only available in the New Horizon Experience. If you open it while still on Legacy Horizon, Horizon explains that you need to switch to New Horizon, then open Shared and Forms again.

Create a form

On All forms, choose Create form to start a blank draft, or Create from template to open the template gallery and start from a best-practice form. You can also choose Import backup to load a Horizon form backup JSON file as a new draft.

The form builder opens in a window. Work through the steps in order:

  • Setup - form title, optional internal name, instructions, how the form is used, and related options

  • Fields - add questions from the field-type list

  • Logic - show or hide fields from answers (Forms Plus)

  • Pages - group questions across pages

  • Completers - who must complete the form

  • Notifications - email staff when an answer matches a value (Forms Plus)

  • Confirmation - success message and optional receipt email

  • Publish - make the form live for members

  • History - restorable versions of the form

Use Preview to walk through the form without saving answers. Use Test (or Test submit) to submit a test response that does not count toward mandatory completion or reminders and never sends notifications.

How the form is used

On Setup, How this form is used chooses the attachment:

  • Standalone - members open it from the Forms hub or a login reminder

  • Attached to a game - members complete it for a specific game from their schedule

Standalone forms can require sign-in before opening, and can repeat once, once per season, once per year, or every time. Game forms can be mandatory or optional for the assigned crew. Staff forms opened from Master Schedule are always optional; to require staff completion on a game, assign them in a crew role and use an assigned-crew form.

Who completes the form

On Completers, choose who the form is for:

  • Assigned officials - accepted crew on Personal Schedule, with optional league, game code, and position targeting

  • Staff form - organization administrators, schedulers, or observers on Master Schedule

  • Coach Schedule roles form - coaches, athletic directors, League Personnel, and location personnel

For officials on specific games, choose a league when your organization uses leagues, a game code, and a position. For staff or Coach Schedule roles on specific games, choose at least one league or game code.

Publish

When the draft is ready, open Publish and confirm Publish this form? Members can then complete it. Publishing increases the form version when questions or answer options change on a live form. Use Unpublish from the form's more-actions menu before you can delete a published form. Archive stops new submissions while keeping existing responses for staff.

Review responses

On a published form card or row, choose Responses. The responses board shows submission counts, completion, needs review, and escalated totals, plus charts and the response list. Search responses, filter by workflow status, set a date range, and choose whether to include test submissions.

Open a row with View to read answers, add an internal note (staff only; not shown on member PDFs), and update workflow. Workflow statuses are New, Reviewed, and Escalated. You can mark several responses reviewed at once with Mark reviewed. Use Export CSV, Export PDF, or Print when you need a file or a printed copy. Export is labeled Export CSV here because the board also offers PDF export.

With Forms Plus, the responses board also offers Horizon AI Summary after at least three real submissions. The summary is a briefing of themes in the answers; generate it from the summary control on the board.

Compliance and reminders

Open the Compliance tab to see incomplete mandatory standalone forms and incomplete game forms for assigned positions and Coach Schedule roles. The board lists who still needs to finish, with overdue and due-soon cues when a deadline applies.

Select people and choose Remind now (or Remind) to send one reminder email per person. Horizon allows one manual reminder per person per form per day. Daily reminder emails also appear under Reminders after sign-in when something is due. Draft and unpublished forms never send reminders.

Complete a form as a member

When a form needs your attention, open Forms and check Due. Optional forms sit under Available. Finished work is under Completed, where you can view your submission or download your PDF when that is offered.

Game forms also appear from Personal Schedule, Master Game Schedule, or Coach Schedule under the form's action name, such as Complete followed by the form title. Fill each page, then choose Submit form. If Save and resume is on for that form, you can leave and come back to a draft. If Allow edit after submit is off, only an organization administrator can reopen your submission.

Forms Plus

Forms Plus is the paid upgrade for Custom Forms. It raises the published-form limit and unlocks:

  • Conditional logic (show or hide fields from answers)

  • Notifications that email staff when an answer matches a value

  • Horizon AI Summary on the responses board

When a step needs Forms Plus, the builder shows Forms Plus required and an Unlock Forms Plus path. Test submissions never send notifications, even with Forms Plus.

If you still see the older layout

Custom Forms is only available in the New Horizon Experience. On a computer or laptop, the older legacy layout appears only if you switched back to Legacy Horizon. Choose Switch to New Horizon from the banner or your account menu, then open Shared and Forms again.
